Important Information for Parents

 All required forms are to be returned to your child’s teacher by Thursday, September 17, 2015

 Forms that need to be returned to the school:

  1. Data Verification Form-The black and white computer form will come home the first week of school. Please check to make sure that all of the information is correct. Change anything that is incorrect in red. Remember to SIGN it on the last page before returning it.
  2. Notification and Permission for the use of Student Personal Information Form We do not share any personal information with anyone outside of the school. Parents/guardians of students under the age of 18, are to be informed of the uses of their personal information. We want to know that all of our students are safe each day. We ask that you call every time your child is absent. If you don’t call, that is our signal to start looking for your child(ren). The Attendance Check phone number is 519 570-8051 Mailbox 3896#. Please clearly state the name of your child and their teacher and the reason for the absence when you call. Parents please call each day your child will be absent or late.
  3. Medically Fragile updates will be sent home Tuesday September 8, 2015. Please return these forms as soon as possible.
  4. Student Accident Insurance – OPTIONAL – During the course of the school year, our students have many opportunities to engage in various learning activities. Specific courses such as our science and technology classes, that involve the use of tools, chemicals and machinery, and our physical education classes, that involve movement with equipment, carry a risk of injury. In addition, students participating in the interscholastic athletics program or field trips should be aware that there is a risk of injury when engaging in those activities. Sometimes, accidents result from the nature of the activity and can occur without any fault of the student, or the School Board or its employees or agents, or the facility where the activity is taking place. The Waterloo Region District School Board does not provide any accidental disability, dismemberment, dental, death, or medical expenses or insurance on behalf of the participating students. Parents are asked to refer to Staebler Accident Insurance website for online subscription at:

  1. Permission for EMAIL communication – School boards continue to be subject to Canada’s anti-spam legislation (CASL). Parents are encouraged to enter your email using our online form at https://secure.wrdsb.ca/subscribe
  2. School-Day –Visit www.school-day.com for ongoing information as we continue to update it.
  3. Drop off and Pick up – All students are encouraged to walk or ride to school if possible. We are looking for crossing guard volunteers to support students crossing at the Laurelwood Drive crosswalk. Please let the office know if you are interested. Students arriving by car can be dropped at the Kiss and Drop along Laurelwood Drive (no stopping or leaving your vehicle as per parking By-Law), or may be dropped in the front parking lot at the WEST end of the drive way by the exit to Laurelwood Drive (no parking or stopping). Students will be asked to enter at the REAR of the school.
  4. Safe Arrival and Safe Welcome – WRDSB schools have a safe arrival program in place, which requires all late arrivals to sign in at the main office on arrival. Safe Welcome program at WRDSB schools requires all school doors to be locked at all times. Visitors must ring an entry bell to request entry to the school at the front of the school, and come to the main office on entry. Note – there is a wait associated with this arrival system. Visitors will have to wait to be welcomed in depending on availability of staff.
